Onychomycosis is a fungal infection of the toenails. This creates nails which are thick, yellow, and brittle. The thickness may create athlete’s foot, pain, and bacteria infections. The appearance is also unsightly to most. This in turn can create psychological concerns when in public. Generally one of the toenails is affected first. Then it slowly spreads to the other toenails. As the fungal infection spreads to the entire toenail, it may create an extremely thick nail. This can produce pain, ingrown toenails, and permanent damage to the matrix (toenail root).
The condition generally begins at the very end of the nail. Over a period of time, the fungus grows inward while the toenail grows outward. The causes include injury (the toenail is lifted off of its nail bed), Diabetes, and immune disorders. Psoriasis can produce the appearance of fungus. Psoriasis in most cases presents with pitting and numerous small white areas.
Treatment consists of debriding the affected toenails, topical solutions, permanent removal of the toenail(s), oral medications, and LASER. Oral medication may be used concurrently with a topical solution. This increases the cure rate. The oral medication is prescribed for 3 months. In regards to oral medication, the main concern is the liver. Although there is minimal risk to the liver, a blood test prior to taking the oral medication is indicated. This will evaluate the liver. The manner in which the oral medication works is to become a part of the new toenail and skin. As the new nail grows out, the medication kills the fungus and stops its penetration. Whenever treating this condition, the patient must be aware that it is a long term treatment. This is due to the slow growth of the toenail. In some cases, over one year was required for a cure.
LASER treatment acts to destroy the fungus. This treatment does not require oral medication, blood tests, shots, or soaking. This treatment is performed by a doctor trained to use the laser with good affects.
